Er war Supervisory Aerospace Technologist am Johnsons Space Center in Houston. Der Text gibt eine zusammengefasste Geschichte des Films vom Start am 19. Juli 1969 bis zur Landung am 22. Juli 1969. Unten links befinden sich Faksimile-Unterschriften der Besatzung über dem Apollo 11 Emblem. Das Dokument ist gerahmt und in einem altersgemäßen guten Zustand. Vorderseitig unten rechts ist das Dokument leicht verschmutzt.

It is mounted on a printed certificate about DIN A4 in size, which is signed "Richard W. Underwood". He was a Supervisory Aerospace Technologist at the Johnson Space Center in Houston.
